Medical Records

PLEASE NOTE: BEGINNING OCTOBER 1, 2010

The following fees will be charged when requesting your medical records.

    Records for personal use will be billed as follows to your Route Y, or credit card account:
  • 0.25 per page for the first 100 pages
  • $25.00 flat fee for anything over 100 pages
    Electronic Media:
  • $5.00 per disk
    Records requested by physicians for continuing care will be free of charge.

All of your records are confidential.

If you are over the age of 18, your parents cannot have information about your visit unless you sign a release at the SHC Medical Records office 2102 SHC stating that you give them permission to talk to your doctor or for us to send copies of your treatment notes.

We will send your treatment records, when accompanied by a signed release, to another doctor without charge to you (this is considered a professional courtesy).

If you want to have your records requested from another physician's office please fill out this form and fax or mail it to the address/number listed on the top of the form.

If you would like the Student Health Center to send your records to another physician's office, fill out this and fax or mail it to the address/number listed on the top of the form.

As a protected right under HIPAA (Health Insurance Portability and Accountability Act), psychotherapy notes will not be released to the patient as part of the medical record. However, Psychotherapy notes can be sent directly to another clinician for continuing care.
