Respiratory Protection Training Medical QuestionnaireIntroduction: All BYU employees who are required to wear respirators as determined by BYU's Respirator Program Administrator must complete a medical questionnaire (annually) as outlined in OSHA's Respiratory Protection Rule 1910.134. Double strapped dust masks or similar items that are NIOSH approved are considered respirators by OSHA. The defining factor is required use or voluntary use. Please contact (contact information can be found below) the Respiratory Program Administrator for voluntary use options and questions. Supervisor Instructions: Please direct employees to complete and submit the online questionnaire at least three weeks prior to respiratory protection training and fit-testing. Employees who are required to use respirators must have a completed evaluation before fit-testing can be completed and they can officially perform work with a respirator. There are no exceptions to this requirement. Note: This is an annual requirement. Outline:1. Required respirator users complete online medical questionnaire. "See link below"a. Have BYU ID number ready. 2. Reviewing physician (Dr. Donald Fairbanks) reviews and notifies program administrator as to the employee's ability to safely wear the required respirator. a. If necessary, the employee will need to be evaluated further as determined by the reviewing physician. 3. Program administrator contacts employee's supervisor to set up training. 4. Employees attend training, complete fit-testing and are issued respirators. a. Risk Management will provide respiratory protection. Other important items: If your employee's position or tasks are likely to create a real or potential airborne hazards and the employee's exposure has not been assessed by the program administrator, please complete a Respirator Request form which can be found by clicking here.
