Health Plan ID

The Health Plan ID helps students have access to all of the benefits of the Health Plan.

How To Receive ID

• Go to dmba.com
• Select Log In
• Create an account by typing your DMBA member ID# in the USERNAME
• Then go to the bottom of the page and "Create New Account". Then follow the instructions to set up your portal. If it gives you an error telling you ID doesn't match, call DMBA at 800-777-3622.
• After you create your portal, you can take a picture, print, or request a card be emailed or mailed to you.

*You can find your DMBA ID# on your Financial Center home page in the Profile information.

**If you are having problems accessing the mobile website while connected to a public/home network, verify with the network administrator that the network is not limiting your access.
