Health Plan ID

The Health Plan ID helps students have access to all of the benefits of the Health Plan.

How To Receive ID

Step 1: Log in to DMBA.com

  • Go to dmba.com
  • Select "Log In"
  • If you have already set up your DMBA profile, please enter your DMID and password. Proceed to Step 2.
  • If you haven't set up your DMBA profile yet.
    • Create an account by selecting "create new user login".
    • Complete the form by entering your DMBA ID*, First Name, Last Name, BYU Student ID Number,  and Birthdate. 
    • If it gives you an error telling you that your ID doesn't match, call DMBA at 800-777-3622.

 * You can find your DMBA ID# on your Financial Center home page in the Profile information.

Step 2: Locating your digital DMBA ID Card

  • On "My Dashboard," select "My Health" in the top dropdown menu.
  • Under the "Medical" section, select "ID Card".
  • You can save the images to your device, fax the ID card, email it to yourself or others, request a physical card, or print it instantly.

If you need help finding your DMBA ID# or accessing your card, please contact DMBA at 800-777-3622. If you experience issues accessing the mobile website while connected to a public or home network, check with the network administrator to ensure that network access is not being restricted.
